Kate Hodges Prescribing Pharmacist

Gender: Female

About me:

Kate is the Practice Clinical Pharmacist and an Independent Prescriber with specialist expertise in cardiovascular disease prevention and management, including hypertension and lipid optimisation. She conducts comprehensive medication reviews, supports complex prescribing decisions, and provides expert advice to both patients and clinicians. Kate plays a central role in medicines safety, monitoring, and optimisation across the practice, ensuring high‑quality, evidence‑based prescribing.

Before joining primary care in 2022, Kate spent almost 15 years working in community pharmacy, giving her extensive experience in managing minor ailments, supporting self‑care, and delivering frontline patient advice. This background enables her to combine clinical knowledge with a strong understanding of patient behaviour and accessibility.

Within the practice, Kate works closely with the lead partner on medicines management initiatives, contributes to QOF and IIF performance, and supports the safe implementation of prescribing changes. Her work directly improves patient outcomes, reduces medicines‑related risk, and ensures the practice meets national and local prescribing standards.

Kate works remotely and therefore offers telephone consultations, providing flexible, accessible support to patients while remaining fully integrated into the clinical team.
